Student Absences

Please check your syllabus for specific details on what and how absences are processed. Any technical issues with this application can be sent to our Stockroom Manager, Hakme Lee (hakme@uw.edu).
 
Please read your specific course policies on Absences.  This form is centrally managed by the Department of Chemistry for your convenience.  Final approval of absences is determined by your course instructor.

MEDICAL RELATED ABSENCES: Please submit absences no later than 48hr after the absence.  Failure to do so may result in a rejection of your absence excusal.  Please do not send us your personal medical information.  Long term accommodations will require approval from DRS services (see below).

UW SPONSORED ACTIVITIES: UW sponsored activities such as NCAA sports must be submitted at the beginning of the quarter or as soon as you qualify for an event.  Please speak with the staff coordinator, get a signed note, and turn it in ASAP.  CLUB or RSO activities will not be accepted but may be excused at the discretion of the director of Undergraduate Services Eric Camp (ericcamp@uw.edu).

ACCESS AND ACCOMMODATIONS: Your experience in this class is important to us, and it is the policy and practice of the University of Washington to create inclusive and accessible learning environments consistent with federal and state law. ALEKS, Achieve, and Prelab Assignments:  Due to the time students are given to complete these assignments (1 week), excusals for these assignments are only granted in extraordinary circumstances.  Please contact the Director of Undergraduate Services prior to submitting an absence (ericcamp@uw.edu)
