MEDICAL RELATED ABSENCES: Please submit absences no later than 48hr after the absense. Failure to do so may result in a rejection of your absence excusal. Please do not send us your personal medical information. Long term accommodations will require approval from DRS services (see below).
UW SPONSORED ACTIVITIES: UW sponsored activities such as NCAA sports must be submitted at the beginning of the quarter or as soon as you qualify for an event. Please speak with the staff coordinator, get a signed note, and turn it in ASAP. CLUB or RSO activities will not be accepted but may be excused at the discretion of the director of Undergraduate Services Eric Camp (ericcamp@uw.edu).

ALEKS, Sapling, and Prelab Assignments: Due to the time students are given to complete these assignments (1 week), excusals for these assignments are only granted in extraordinary circumstances. Please contact the Director of Undergraduate Services prior to submitting an absence (ericcamp@uw.edu)

- Missing two labs (not including the lab safety orientation) that are excused will automatically trigger an incomplete. You only need to make up one of the two the following quarter. Please contact ericcamp@uw.edu if you have any questions about the process.
- Missing multiple exams or the final that are excused may trigger an incomplete. Please reach out to your instructor if you have concerns about the process.
